Dr. Jim Feng is a graduate of University of Toronto with a Physical Health and Education Degree. From there, he attained his Doctor of Chiropractic from the Canadian Memorial Chiropractic College. He has also completed theory levels 1 & 2 of the National Coaching Certification Program and is a Certified Strength and Conditioning Specialist through the National Strength and Conditioning Association, Sport First Responder, and Certified Titleist Performance Institute Medical Professional. Dr. Jim Feng has made an extra effort in extending his knowledge in the human body by acquiring the Active Release Techniques (ART®) certification, as well as the Medical Contemporary Acupuncture provider designation through McMaster University. Dr. Jim Feng has always maintained a passion for exercise and health. As the Captain of the University of Toronto Varsity Wrestling team, Dr. Jim Feng utilized his knowledge in functional, dynamic, power, endurance, plyometrics and other sport specific training to lead his team to winning provincial, national and international tournaments. As a Chiropractor, Dr. Jim Feng has a wide and varied knowledge of therapeutic rehabilitation and exercise prescription which fits closely with the Dynamic Treatment Paradigm. With his extensive knowledge in rehabilitation and sports specific training, he was selected to lead medical teams for multiple Olympic Trials , International Grappling, World Amateur MMA and Pancration Championships, UFC events as well as the head therapist for Team Ontario, Wrestling and Canadian Grappling Championships. Dr. Jim Feng’s upbeat attitude and vibrant outlook allow him to encourage and motivate his patients in a fun environment. His strong knowledge and solid expertise make patients feel confident in his abilities to empower them to achieve their functional goals.
Ada Chiu
Chiropodist
Ada graduated from the University of Toronto with a Bachelors of Science degree in Human Biology. She then received her Chiropody training at the Michener Institute for Applied Health Sciences in Toronto, where she underwent 3 years of practical training in podiatric medicine, biomechanics, orthotic therapy, anesthesia, nail surgery, physical therapy, and podopediatrics. Ada is a registered member of the College of Chiropodists of Ontario, member of the Canadian Federation of Podiatric Medicine, and member of Prince Edward Island Podiatry Association. As a foot care provider, Ada believes that chiropodists can help people of all ages. In children, early detection of many foot problems such as flat feet, in or out-toeing can prevent serious complications from developing later in life such as knee and lower back pain. The geriatric population can also benefit from chiropody services for the treatment of painful lesions resulting from the effects of diabetes, rheumatoid or osteoarthritis, and biomechanical anomalies. Ada is excited to be part of the Form and Function team. In her spare time, she likes hiking and recently takes up baking. Ada is fluent in Cantonese and looking forward to bringing the best of foot care to you and your family.
Afshin Shams
Physiotherapist
Afshin’s treatment philosophy involves a comprehensive approach, treating beyond structures and looking at the system to identify sources of nociception (painful stimuli). Afshin’s practice focuses on the use of manual techniques and neurofunctional electro-acupuncture in addition to prescribed home exercises for the rehabilitation and prevention of sports injuries and pain with movement disorders. Afshin received his Bachelor of Science in Kinesiology from York University. He then went on to complete his Physiotherapy degree at the European School of Physiotherapy in Amsterdam, Netherlands with special interests in orthopaedic and sports rehabilitation. Afshin has completed post-graduate courses in Contemporary Medical Acupuncture from McMaster University as well as the Advanced Integrated Musculoskeletal (AIM) program through the Orthopaedic Division of the Canadian Physiotherapy Association.
Dr. Christopher Duong

Dr. Chris had always questioned about the human body. Ultimately, his fascination led him to achieve his Doctor of Chiropractic at Palmer College of Chiropractic West. Dr. Chris has a strong passion for helping individuals who want to feel and perform better, whether it is from resolving pain or helping to progress the quality of health. What he enjoys most about his profession is the doctor-patient interaction, and believes that building a strong relationship with patients will help empower the patients to optimize their well-being. Credentials: RockTape FMT and PMT Certified, Functional Range Release, Dynamic Neuromuscular Stabilization Exercise 1, Selective Functional Movement Assessment Level 1.
Fang (Ada) Lu

To prepare for her massage career, Ada graduated from Pharma-Medical Science College of Canada in January 2021. Since then, she has been studying acupuncture and tuina massage. Ada is pleased to offer every client professional care in a beautiful, relaxing environment. Offering an array of methods Ada believes that the combination of Gua sha, hot stone and soft tissue release, through massage therapy, are integral aspects for maintaining optimal health and balance to the body and mind. She adapts every massage therapy treatment to each individual, helping create this balance for her clients. Ada’s focus is on Attachment Release, Trigger Point Therapy, and Deep Tissue Release, utilizing her elbows and knuckles when needed. While working with deep tissue, Ada slowly applies pressure, minimizing discomfort and allowing the body to relax completely. Treatments range from specific work on chronic areas and injuries, to full body muscle, tension, and stress reduction. Outside the practice when Ada is not in the office, she loves to go on adventures with her two daughters and dog. Pre-pandemic, Ada enjoyed traveling and trying different foods.
